Dear valued patient,
It is with mixed emotions that I have made the decision to close my practice on July 28, 2022 and will be relocating to Florida. It has been an honor and a privilege to be a part of your healthcare, and it is very important that you continue to receive gynecologic care with a new provider.
Part of the smooth transition of care includes making sure that you have access to your patient chart. As our office uses Electronic Medical Records (EMR) through the company Practice Fusion, your electronic medical record is stored in the cloud, thus there is no physical paper chart. We are asking that you log in to your patient portal so that you will always have full access to your chart. You were sent an email invite at your first visit with our office, but if you need a new invitation, we will be happy to send one. You will need to include a correct email address on the return form so that we can email the invite to you. You can also visit MyQuest at https://myquest.questdiagnostics.com to have access to your lab results. Also, we are happy to electronically fax your treatment records to the new gynecologic provider that will care for you. To do this, you will need to sign the enclosed medical release form with your new provider’s name and address. After I receive this information, I will forward your records.
Choosing a new provider can seem challenging. We recommend you begin your search as early as possible. You may contact your insurance plan for a list of in-network providers, or you may choose from the brief list of female providers below.
